How Police HomeCare collects, uses, stores and protects your personal information.

Last updated: September 2025.

Police HomeCare ("we", "us", "our") is committed to protecting your privacy and handling your personal information in an open and transparent way. This policy explains how we collect, use, disclose and protect personal information, and applies to everyone we deal with, including clients, participants, families, carers, staff, volunteers and job applicants. We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

The information we collect

The personal information we collect depends on our relationship with you. It may include your name, contact details, date of birth and emergency contacts; health and medical information relevant to your care; My Aged Care or NDIS details, including reference and plan numbers; funding, billing and payment information; and, for job applicants and staff, your employment history and related records.

Some of the information we collect is "sensitive information", including health information. We only collect sensitive information with your consent, or where we are required or permitted by law.

How we collect it

We collect personal information directly from you wherever possible, for example when you enquire, register for services, complete forms, or speak with our team. With your consent, we may also collect information from your family, carers or advocates, from health professionals involved in your care, and from government departments such as My Aged Care, the National Disability Insurance Agency and Services Australia.

How we use your information

We use your personal information to plan, deliver and coordinate your care and support; to communicate with you and the people involved in your care; to process funding, billing and payments; to meet our obligations to regulators and funding bodies; to recruit and manage staff; and to improve the quality and safety of our services. We do not sell your personal information.

Disclosure of your information

We may share your information, with your consent or where permitted by law, with support workers and clinical staff involved in your care; with family members, advocates or support coordinators you have nominated; with health professionals and other providers involved in your care; and with government departments and funding bodies. Where we engage third party service providers, we take reasonable steps to ensure they protect your information.

Keeping your information secure

We take reasonable steps to protect your personal information from misuse, interference, loss and unauthorised access, modification or disclosure, through physical, electronic and procedural safeguards. We retain records only for as long as required by law and our funding obligations, and dispose of them securely when no longer needed.

Accessing and correcting your information

You have the right to ask for access to the personal information we hold about you, and to ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, please contact us using the details below. We will respond within a reasonable time.
